A new study by the Congressional Budget Office (CBO) shows that physician-led ACOs are the most effective at reducing healthcare costs. I love the chance to spend time with healthcare’s changemakers — the people doing the work to make value-based care a success. Last week’s National Association of ACOs (NAACOS) conference let me do just that. I shared the stage with Wilson Gabbard, Eric Cragun, Tori Bratcher and Jennifer (Albee) Houlihan, MSP to talk about how our organizations are translating care models from Medicare Advantage to MSSP, REACH, and other ACO models. The common thread that became clear between our presentations was the importance of simplifying the care model for PCPs, allowing them to be payer-agnostic and treat patients in the same way. What also became clear was that although metrics and expectations for separate payors (Medicare vs. Medicaid) or programs (Medicare Advantage vs. ACO REACH vs. MSSP) may be different, they are applicable across populations and programs to deliver better outcomes for all patients. Administratively, there are meaningful differences between Medicare Advantage, MSSP and REACH ACO contracts. But unifying the care model across all government programs leads to population-level outcome and patient experience improvements while decreasing costs – i.e., it generates value. When I practiced medicine at Oak Street Health, most of our patients were in Medicare Advantage. So, we did not have the complexities of variable contracts and plans; it was ideal. Organizations that work to achieve that level of synergy across varying contracts will likely experience economies of scale in operations, aligned PCP buy-in and clinical care, and ultimately better patient outcomes. How do those outcomes come about? We know that more than half of what impacts patients’ outcomes are social drivers of health and their physical environment. The other big driver is health behaviors outside of direct healthcare delivery. So, a team-based approach between providers and the clinical workforce for a care model that involves care managers, social workers, and other non-clinical, patient-facing roles is imperative to creating value. The other issue, however, is that many of us don’t learn how to effectively collaborate with these professionals in our medical education – either in medical school or residency. But, they are foundational in the care model to appropriately care for patients, make their lives better, and as a result, bend the cost curve. At Medical Home Network (MHN), for instance, we help fund FQHCs in our ACOs to hire and train these indispensable care team members. In the 2024 State Health Disparities Report from the The Commonwealth Fund, data shows there are still profound racial and ethnic disparities in health, well-being and life expectancy in the U.S. One key factor cited is unequal access to primary care.
